Registration Information

To register a Preschool student(s) for OLPH school you need to do the following:

1) Download the form, fill it out and bring it in to the school office.

2) Call the school office (419-382-5696) and make an appointment to go over your forms and drop off a check for the registration fees.

1) Fill out the registration form below OR download the form, fill it out and bring it in to the school office.

2) Call the school office (419-382-5696) and make an appointment to go over your forms and drop off a check for the registration fees.
