Registration Information
OLPH School admits students of any race, color, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, national and ethnic origin in administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs.
To register a Preschool student(s) for OLPH school you need to do the following:
1) Download the form, fill it out and bring it in to the school office.
2) Call the school office (419-382-5696) and make an appointment to go over your forms and drop off a check for the registration fees.
To register a K-8 student(s) for OLPH school you need to do the following:
1) Fill out the pre-registration form below OR download the form, fill it out and bring it in to the school office.
2) Call the school office (419-382-5696) and make an appointment to go over your forms and drop off a check for the registration fees.
Please note, Registration is not considered complete until the following has been done:
1) we have met with families in person.
2) registration fees have been paid.
